To get prescription reimbursement, you need to go to your drugstore and get a print out of your prescriptions. Once you have your print out, you need to mail it in to us at: EBS P.O. Box 13749 Jackson, MS 39236 You can also fax them into us at: 601-353-0004.
No, your insurance card does NOT have an expiration date.
If you do not have your Employee Benefit Services card with you at the time of service, all you need to do is inform the provider of your coverage and give them our telephone number (601) 353-0002. They can call us to verify your coverage and any other questions they may have.
The benefits for registering online are great. For both the INSURED and the PROVIDER, claims information, benefits & eligibility, and the effective date are all available 24/7. Also online, extra EOB’s can be printed out for your records.
